Respected Sir, I am Irshad Khan working in Modison Limited Metals, Vapi GIDC under contractor Mahadev Industrial Services. On 10/07/2026 at 6:50 PM my left hand 4th finger tendon was cut in machine during work. Contractor is not giving me Accident Report Form-16 and abusing me. Due to non-availability of Form-16 my TDB and treatment is pending. I have already given written complaint to ESIC Hospital Vapi. Kindly help me to get Form-16 and start my TDB.
